Professional oral hygiene in the dentist’s office helps to ensure comprehensive dental care and to detect problems in time. It is a preventive measure against diseases of the teeth, gums and mouth.

Professional oral hygiene is a procedure that removes supragingival plaque, subgingival plaque that is not visible to the naked eye, superficial extrinsic stains, and polishes the tooth surfaces. At the same time, pathogenic micro-organisms accumulated in plaque and the toxins they produce are eliminated and bad breath disappears.

During this procedure, the importance of individual oral hygiene at home is explained to the patient, and the right oral hygiene products are selected and taught to be used correctly.

Who needs professional oral hygiene?

 

Oral hygiene in the dentist’s office is necessary because patients are not able to clean all the plaque that accumulates on the surface of their teeth and gums with simple home-made products. They are also unable to detect some incipient oral diseases. Poor nutrition and hygiene skills lead to an imbalance of ‘good’ and ‘bad’ bacteria in the mouth, to the development of tooth decay and bleeding gums. In addition, dangerous micro-organisms from oral plaque can enter the bloodstream (bacteraemia) and worsen a person’s overall health.

So, we would like to emphasise that oral hygiene has two main goals: cleaning and prevention. Although most patients come for the former, it is very important not to underestimate the latter. Any preventive action, investing in preventive procedures, is always better than treatment. Emotionally, in terms of time and financially. For example, if you don’t take proper care of your gums in time, you may have to say goodbye to one tooth or another. And dental implants are not only more time-consuming but also more expensive than oral hygiene.

How is professional oral hygiene performed?

Professional oral hygiene is carried out using a wide range of professional dental tools, instruments and materials. Its duration depends on the complexity of the situation. The more plaque that needs to be removed, the longer the procedure takes. It usually lasts between 30 and 60 minutes.

Professional oral hygiene at the Dantų meistrai clinic in Kupiškis is carried out using the same tools, instruments and methods as in the country’s major clinics.

 

The main steps in the oral hygiene procedure:

  • local anaesthesia in case of tooth sensitivity;
  • removing tartar above and below the gums using an ultrasonic scaler;
  • special curettes, hooks (hand instruments) are used to remove tartar and soft plaque deep in the gum pockets, between crowded teeth or around existing dentures, implants;
  • The “Air Flow” system removes pigmented stains caused by smoking, black tea, coffee and other staining foods. It is safe for children and adults to use, as there is a controlled flow of soda or calcium carbonate particles that are directed to the tooth surface by a jet of water and compressed air. In the clinic, we use calcium carbonate powder (Kavo Prophy Pearls), which has rounded particles, so that dental plaque is cleaned away, sparing the tooth surface and the soft tissues of the mouth. The “Air Flow” method removes plaque from uneven tooth surfaces, depressions, interdental spaces and smooth tooth surfaces. Cleaning is carried out at a distance, so that the hard tissues of the tooth and gums are not damaged, reaching even the hardest to reach areas;
  • for cleaning implant-supported dentures, zirconia and ceramic dentures, we use water-soluble baking soda, which is non-abrasive and does not damage the denture surface;
  • after cleaning the plaque, the tooth surface is polished with special brushes, pastes and erasers. These polished tooth surfaces are more resistant to the build-up of plaque and tartar. In addition, the interdental spaces are cleaned with interdental floss;
  • for gingivitis, disinfecting liquids are used to rinse the gingival pockets and polish the sharp corners of fillings;
  • patients are given mouthwashes with special fluids;
  • if necessary, fluoride paste preparations are applied to restore the mineral balance in the teeth. They are effective in treating early-stage tooth decay, while protecting healthy teeth from caries and increasing the enamel’s resistance to acids produced in the mouth by micro-organisms when carbohydrates are decomposed or in cases of gastro-oesophageal reflux disease. They are also involved in enamel remineralisation and prevent plaque from attaching to the surface of the tooth. Fluoride applications reduce or eliminate tooth sensitivity. They are particularly recommended for children during the eruption of permanent teeth, as fluoride helps to strengthen the enamel. If there are multiple caries lesions in the mouth or if the patient wears braces, it is recommended to fluoridate the teeth with varnish or foam to prevent further caries development;
  • the condition of all the teeth and the oral cavity is examined, and the patient is alerted to any problem areas;
  • individual dental care recommendations are given, and the patient is taught how to brush their teeth correctly using different cleaning methods.

During oral hygiene:

  1. Removing hard tartar that cannot be removed with a toothbrush;
  2. Removing soft plaque located in hard-to-reach areas;
  3. Removing pigmented stains on the surface of the teeth caused by smoking, coffee, tea, red wine or other staining drinks or foods;
  4. Preventive check-ups for teeth, gums and oral cavity.

Proper cleaning of dental plaque prevents bacteria and micro-organisms from forming on the surface of the tooth for longer, which can lead to oral diseases such as gingivitis, carious enamel lesions, which can lead to the growth of bacteria and the development of periodontitis. Professional oral hygiene for children

As soon as the first tooth appears, brushing should already have started. There are special toothbrushes and toothpastes in pharmacies for the little ones. However, no matter how hard you try, it’s not always possible to clean your little teeth thoroughly. Some are reluctant to yawn, others try to brush only by themselves, even though it doesn’t work very well, and still others find the ritual very unpleasant at all, culminating in shrieking. Professional oral hygiene for straightening teeth

For patients wearing different types of braces, professional oral hygiene is recommended between visits to the orthodontist because of the burden of brushing. Even when brushing with a single tooth or interdental brush, patients need very good manual skills to remove the soft plaque that has formed. Very often, not only soft plaque but also food debris is trapped in the inner part of the bracket, close to the orthodontic wire and the tighteners, thus significantly increasing the risk of caries or discolouration around the bracket.

The Icon caries infiltration system, developed by the world-renowned German manufacturer DMG Dental, allows to strengthen the demineralised enamel layer without decay, preserving healthy tooth tissue and thus stopping the spread of caries in time.During the procedure, a special material is applied on the surface of the tooth, opening the pores in the surface of the damaged enamel. After washing the treated tooth surfaces with ethanol and air-drying, a liquid high-penetration composite resin is applied to the tooth. Under the influence of capillary traction, the open enamel pores “absorb” the liquid resin, which is then cured by light. This strengthens the demineralised enamel layer and stops the penetration of caries into the deeper tooth tissues. The procedure, which takes about 30 minutes, is painless, causes no discomfort and the results are visible immediately.

With the Icon infiltration system, the spread of early caries is stopped in a single visit and white spots of enamel demineralisation that cannot be removed by other means are eliminated.
